In order to keep our costs low we would appreciate payment of any co-pays and deductibles at the time of the initial service. Visa, MasterCard, Discover and American Express are accepted. Our fees are sensible and responsible to the guidelines set by the insurance companies and government agencies. Our office is fully computerized and our staff can help you to obtain the maximum benefit from your personalized healthcare.
You are responsible for the deductibles, co-pays, and the balance after insurance reimbursement and adjustments as specified by your individual plans.
We also make special accommodations for self-pay patients who do not have insurance based on a fee schedule and the nature of the patient’s problem. PPO/Managed Care Contracts
